Position Summary:
We are seeking a dedicated Clinical Nurse - Ambulatory Navigator to coordinate and manage the care of assigned patients within our neuro -oncology department. In this role, you will develop personalized care plans, collaborate with multidisciplinary teams, and advocate for patients to ensure they receive timely, appropriate, and effective care across the continuum. The ideal candidate is compassionate, detail-oriented, and committed to delivering high-quality patient-centered care , and has experience with the oncology population. This role will primarily support our Master Clinician, seeing patients with primary brain tumors and brain metastases.
Responsibilities:
- Coordinate the care needs of assigned patients and develop comprehensive, individualized care plans.
- Partner with patients and their primary physicians to ensure care aligns with patient needs and preferences.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to facilitate timely testing and treatment.
- Advocate for patients, balancing clinical requirements with financial considerations to promote efficient care delivery.
- Ensure compliance with healthcare regulations, policies, and documentation standards.
Qualifications:
- Associate's Degree in Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
- Valid and active RN license in Indiana or valid Nurse Licensure Compact (NLC) RN license.
- Graduate of a nationally accredited nursing program.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).
- 3-5 years of clinical nursing experience preferred.
- Knowledge of medical terminology, healthcare regulations, and medical record requirements.
- Proficiency with computer software including word processing and database applications.
- Additional specialty certifications may be required depending on departmental needs.
