Job Title: Clinical Nurse - Ambulatory Navigator
Location: University Hospital
Schedule: Full Time Day Shift; Monday-Friday 8:00 AM- 4:30 PM

Position Overview
The Clinical Nurse - Ambulatory Navigator coordinates the care needs of assigned patients, developing comprehensive plans to manage their care delivery across various settings. This role partners closely with patients and their primary physicians to create tailored care plans based on individual needs and preferences. The Navigator collaborates with physicians, nurses, social workers, and other health professionals to ensure timely and appropriate testing, treatment, and follow-up. The role advocates for the patient, balancing care quality with financial considerations to promote efficient, effective care.
Key Responsibilities:
- Coordinate and manage patient care needs across the outpatient continuum.
- Develop individualized care plans in collaboration with patients and primary providers.
- Facilitate timely delivery of tests, treatments, and follow-up services.
- Advocate for patient needs, ensuring clear communication and support.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Balance clinical needs with resource utilization and cost-effectiveness.
- Ensure compliance with healthcare regulations, policies, and documentation standards.
Qualifications & Requirements
- Associates of Nursing (ASN) required.
- Bachelor of Nursing (BSN) preferred.
- Active RN license in Indiana or Nurse Licensure Compact (NLC) RN license required.
- Graduate from a nationally accredited nursing program.
- 3-5 years of clinical work experience preferred.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).
- Additional advanced life support or specialty certifications may be required per unit/department policies.
- Proficiency in using personal computers, word processing, and database software.
- Strong understanding of medical terminology, medical record requirements, regulations, and policies.
- Excellent clinical assessment and care coordination skills.
- Effective communication, advocacy, and collaboration abilities.
