Position Highlights:
- Shift Schedule: Full-time days, including some holidays.
12 hour weekdays, with 10 hour alternating weekends.
Week A : Monday, Tuesday, Friday, Saturday
Week B: Sunday, Wednesday, Thursday

Pay Range
$37.50 - $56.25

What You'll Need:
- BSN Degree Required/MSN Preferred. Current licensure to practice as a Registered Nurse in North Carolina required.
- At least 5 years of bedside nursing experience. Will consider 4 years if ED/ICU nursing experience
- Certification in the appropriate specialized field within 2 years of being in the position required.
- Excellent communication skills required.

- Ability to work independently
Job responsibilities include, but are not limited to:
- Meeting with patients in the hospital to explain Hospital at Home Program, obtain consents
- Helping to coordinate necessary medications and equipment patients will need for at home care, assisting in coordinating transportation. Ensuring smooth transition from hospital to home environment.
- Coordinating with Case Management team, Attending Physician
- Coordinating the patient's care to increase the efficiency and quality of care that is delivered to the patient
- Serving as the patient advocate and caring for them in a highly supportive and personal manner
- Serving as a liaison to ensure that patients and families receive resources and services as needed.
- Providing language specific education, psychosocial support, and necessary resources for newly diagnosed patients pertinent to their specific disease, and their families (language specific), and coordinating appropriate referrals for those needed services.
- Educating and providing information to new and existing patients and families regarding diagnosis; work-up; treatments to include surgery, radiation therapy and chemotherapy; support services, financial issues; community resources; survivorship issues and follow-up.
- Serving as liaison for patients and families between departments.
- Ensuring and coordinating (where appropriate) new patient referrals, scheduling of exams, procedures and appointments, identifying potential gaps as well as the coordination of care for patients returning to their communities for treatment and follow-up.
- Contacting every newly diagnosed patient at the time of the diagnosis.
- Assessing every new patient for emotional and social needs as well as barriers to care such as: Health insurance, transportation, etc., and refers to the needed resources.
- Identifying trend barriers to patient expedient care and conducting service recovery as needed.
- Responsible for patient satisfaction.
Physical Requirements
- Ability to lift objects weighing up to 25 pounds. Occasional stooping, kneeling, crouching. May require sitting/standing for long periods of time.
