In support of the Indigenous Primary Health Care Advisory Panel’s final report “Honouring our Roots: Growing together towards a culturally safe, wholistic primary health care system for Indigenous peoples” the NMC primary care team will seek to actively address the recommendations as identified by the panel including Culturally safe and approach care; Access; Integration; Quality and Indigenous Peoples as partners.
Through collaborative practice the NMC primary care team will also serve with the following Dynamic Values: Culture-based; Strength-based; Community-based; and Quality-based.
The NMC supports NPs, other healthcare, and administrative professionals in delivering high quality, coordinated care with the aim of enhancing the patient care experience within a community care setting. As well, the NMC ensures enhanced capacity for their care team to work in a positive, hopeful environment which provides their patients and themselves with the opportunity to optimize health, quality of life, and well-being.
The NMC supports the following professional objectives: increase the proportion of patients with ready access primary care in a community setting; provide coordinated and increased access to appropriate primary care services; increase the emphasis on health promotion, disease and injury prevention, and care of the medically complex client who is considering or requiring enhanced support; improve coordination and integration with other health and community care services to enhance and better support the primary care patient throughout their care trajectory; and facilitate the greater use of interdisciplinary teams to provide comprehensive care.
At present, there are 5 priority initiatives identified for the NMC: Prevention and screening; Chronic disease management; Obstetrics; Paediatrics and Indigenous healing practices. Associated program elements within these priority initiatives will align with Samson Health Commission’s overarching goals and objectives: Access; Culturally Safe and Appropriate Care; Integration; Quality; and Indigenous Peoples as Partners.
