Description
The City of Virginia Beach Department of Human Services is seeking to hire a Registered Nurse with experience working with adults with serious mental illnesses in a community-based program for our Program of Assertive Community Treatment (ACT) program. The desired candidate will have experience providing nursing services to consumers, assisting with the management of medication protocols and working closely with members of a team.
Duties:
- Assess/check-in consumers in preparation for physician visits, administration of oral and IM medications, transcription of physician orders, order and package medications, and provide education/counseling regarding medications and relevant nursing problems.
- Transcribe medication orders to the MAR and provide oversight of MAR completion to ensure accuracy and avoid errors in the program. Provide guidance to teammates as appropriate to correct MARs.
- Assist in the provision of medical services, such as delivering and monitoring medications, scheduling and accompanying consumers to medical appointments, collaboration with medical providers and requesting medical records.
- Perform comprehensive assessments for persons with chronic mental illness and co-occurring disorders to determine service, supports and other needs to assist consumers with community stability.
- Develop treatment plans with participation of the consumer and/or guardian and facilitates clinical case reviews and completes all required clinical documentation within agency standards.
- Provide linkage, monitoring, and coordination of services, including medical, social, educational, housing, benefits, and assists consumers with attending necessary appointments to ensure needs are met.
- Provide emergency assessment, treatment and collaboration during work hours and via the ACT emergency phone rotation.
- Provide supportive counseling in 1:1 and group settings to assist consumers with the development of improved health/wellness, coping, problem-solving and communication skills related to treatment goals.
- Provide training and support with activities of daily living, performs crisis intervention, and facilitates prescreening/hospitalization when necessary.
- Assist and participates in all hospital diversion, hospital care coordination, and hospital discharge/re-integration services.
- Serve as the primary care coordinator (SAI) and performs the required duties for an assigned caseload of ACT consumers.
- Maintain clinical records in accordance with regulatory standards and ensures protection of consumer rights and confidentiality and utilizes an electronic health record.
- Participate in team meetings, clinical staffing’s and supervision (individually and in group).
A $5,000 hiring bonus is being offered for this position.Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.
- $1,250 will be awarded in the first paycheck.
- $1,750 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
- $2,000 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.
This position is also eligible to receive a $6,000 annual stipend to be paid in installments over the 26 pay periods

Requirements
MINIMUMS: Requires current and valid licensure as a Registered Nurse by the State of Virginia or as a part of the Compact Designation (Multi-State Privileges) and 2 or more years of clinical experience to be specified by the hiring department. Current CPR certification. Must have a current and valid driver’s license.
SPECIAL REQUIREMENTS: All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. This position may require that incumbents to wear and maintain appropriate personal protective equipment such as, but not limited to, safety footwear, hard hats, safety eyewear, gloves, or other safety attire and equipment in designated areas of risk. Specific requirements will be determined and communicated by the employees supervisor based on position assignments.
