What You Will Be Doing
The successful candidate’s primary responsibilities will be for providing quality care to patients in their homes, assessing patient needs and following care plans, administering medications and treatments, and providing education to patients and their families.
Overall responsibilities for the Registered Nurse include the following:
- Providing quality care to patients in their homes, including following care plans, case management/documentation, transitions of care, and care coordination and continuity.
- Providing wound care, post-op care, medication management and administration, etc.
- Managing a case load of patients
- Performing OASIS assessments, admission, recertification, and discharge visits
- Serving as the point person for patient/family communication
- Building and maintaining a discharge network
- Tracking outcomes, ensures timely documentation, and helps with billing compliance by accurate charting
- Coordinates with other disciplines (PT, OT, ST, HHA)
This individual must exhibit strong compassion and a servant’s heart. We are looking for someone with the ability to lead by example, who is detail-oriented and a self-starter, and has strong written and verbal communication skills.
This is a PRN role: pay is $55.00 – $110.00 per visit (based on visit type and complexity)
Important Notes: This position requires travel via your personal vehicle, which will vary based on business needs. Don’t worry—mileage will be reimbursed.
We are open to nurses looking to relocate. Must have a Nurse Licensure Compact (NLC) license if out of state.
This is not a fully remote/offsite position.
Requirements
Education: Graduate of an accredited School of Nursing
Licensure: Current Texas state license as a Registered Nurse, current Texas Driver’s License
Experience: Minimum one year as a Registered Nurse in home health. REQUIRED.
Experience: One year as Registered Nurse in past 36 months.
Other: Reliable transportation and proof of valid auto insurance
Preferred Qualification:
Experience: Certification in OASIS
