Wound Care Nurse Practitioner or Physician Assistant
Location: HonorHealth Tempe Medical Center & Osborn Wound Clinic
Full-Time | First Assist Required or Willing to Obtain Within 2 Years
HonorHealth, a nine-hospital non-profit health system serving the greater Phoenix area, is seeking a dedicated and collaborative Nurse Practitioner (NP) or Physician Assistant (PA) to join our Wound Care Service Line. This is a multi-faceted role involving inpatient and outpatient care, as well as First Assist responsibilities in the OR.
This advanced practice provider will work closely with surgeons and interdisciplinary teams to improve outcomes for complex wound patients across two key locations: Tempe Medical Center and the Osborn Wound Clinic.
Primary Responsibilities:
- Provide comprehensive consultation and follow-up for wound surgery inpatients and outpatients.
- Perform First Assist duties for wound care surgical procedures.
- Triage patients requiring higher-level or specialty care to the appropriate services.
- Ensure timely and accurate documentation and charge entry (by end of day; minimum standard within 3 business days).
- Collaborate on safe discharge planning and care coordination to reduce readmissions and length of stay.
- Actively promote the wound care service line across the HonorHealth network and community.
- Provide care to clinic patients with surgical needs, including scheduled visits, walk-ins, and coverage for colleagues as needed.
- Partner with clinic staff to meet performance and patient care goals.
Qualifications:
- Licensed Nurse Practitioner or Physician Assistant in Arizona.
- First Assist certification, or willingness to obtain within 2 years of hire.
- Previous wound care experience or work with surgical care patients required
- Strong clinical judgment, communication, and teamwork skills.
